We would like to post abstracts and presentations online so that attendees can reference them and share them with colleagues. Please complete and return the forms below when you submit your session abstract no later than January 15, 2012.

Travel Regulations and Reimbursement
The travel costs for US speakers and organizers will be covered by IUSSTF. Flight arrangements should be made through their corporate travel agency in India, Carlson Wagon-lit Travel, so that your airfare can be direct billed to the Forum. Contact Ms. Meenakshi Rawat at email@example.com
Please book your flights as soon as possible to ensure the best fare.
Your room during the course of the meeting, except for incidentals, will be direct-billed to our hosts beginning with check-in on February 29 and check-out on March 3. The meeting ends at noon on March 3, and we anticipate that all of attendees will return home that afternoon. Please complete the travel form by January 11, 2012. Additional nights are the responsibility of the participant and are not reimbursable.
